SecurityAssist wizard does not generate correct pages - important things missing

When using SA wizard to generate login.php, logout.php, registration.php etc. with my own fields and own database and presets I never get "html", "body", "head" tags nor the script w/ calls to Spry.widget.validationXXX at the end.
In "Manage Pages", if I click to preview the pages about to be created they are correct, even when I visualise the source code: html, head, body tags are present and so the Spry.Validation script.
I use DWCS5 with the last versions of DataBridge and DesignExtender. The issue is the same with my DW CC evaluation version.
I have reinstalled the extensions and deleted the DW cache file many times.
