Form not working

I've had this problem before with go daddy hosting. My form isn't working, it's probably something i'm doing.

Could you check it out to see what's wrong with it? It's not going to my thankyou.php page. I added the phone number field, but I don't believe that has anything to do with it not working.

It's difficult to say what the problem is. There could be a PHP error occurring. try adding the following at line 1:

php:

<?php
error_reporting(E_ALL);
ini_set('display_errors','on');
?>

if that doesn't show any PHP errors, I have created a support ticket for you, please update your ticket by visiting your support history:

I added the line, but I do not see any errors. I just uploaded the form for support.

It doesn't have the line added, I took it out after it didn't show any errors.
