Testing site tax doesn't calculate correctly one order

I have a couple of test orders (the whole site is in test mode) and all the orders have been calculating tax correctly except for a couple. I'm not sure what to check to see why some are incorrect and others (or most) are correct. Ohio sales tax is 6.5% and I have it entered correctly in the site admin area.

I've attached a couple of screen shots to illustrate.
