Blank Insert Page - Error reporting did not work

Hello, I have an insert page that is showing up blank and I understand that it could be a php error. I did add the following code and still the page is blank

<?php
error_reporting(E_ALL);
ini_set('display_errors','on');
?>

I have attached the file so you may be able to look a little deeper.

Please advise on what I might do next.

your page has nothing within the body tags:

Jason, let me try to recreate the insert page using Data Bridge and I will see what happens this next time. Stand By!
