upload images on update page

Im trying to create upload image area on my update page
i created a file field area
upload folder i want dynamic to
galleries cakes so i added this to upload folder <?php echo $row_HistoryUpdate['HistoryReference']; ?>
file <?php echo ((isset($_FILES["gallery"]))?$_FILES["gallery"]["name"]:""); ?>
default file <?php echo ((isset($_FILES["gallery"]))?$_FILES["gallery"]["name"]:""); ?>
the folder is allready on the sever reflecting the historyrefence id
the image is not uploading to the folder

1) in order for a file to be uploaded, the form encoding type must be set to Multipart/form-data.

in the tag selector, select the form tag to edit the encoding type.

2) in the upload behavior leave the file name setting to the default:
[FileName]
