UE error message on submit

Getting the following error message on submit of form:

Warning: require(Email2.php) [function.require]: failed to open stream: No such file or directory in /home/treebiz/public_html/webassist/email/mailformatting_php.php on line 142

Warning: require(Email2.php) [function.require]: failed to open stream: No such file or directory in /home/treebiz/public_html/webassist/email/mailformatting_php.php on line 142

Fatal error: require() [function.require]: Failed opening required 'Email2.php' (include_path='.:/usr/lib/php:/usr/local/lib/php') in /home/treebiz/public_html/webassist/email/mailformatting_php.php on line 142

The code on line 142 is: require(basename($fileName));

Email files are attached. Email link is: Estimate_email.php

this error means that not all of the files from the webassist/email folder where uploaded to the server.

make sure to upload the entire webassist/email folder to the server.

Here are the files and subdirectories inside the webassist directory on the remote server:
class.phpmailer.php
class.smtp.php
mc.zip
ncpf.php

email subdirectory:
mail_php.php
mailformatting_php.php
waue_Estimate_email_1.php

language subdirectory:
phpmailer.lang-en.php

What's missing?

in the /webassist/email/waue_Estimate_email_1.php

the following code is looking for the /Email2.php file
$MailBody = $MailBody . (GetFromPage("../../Email2.php"));


the error is saying that the /Email2.php file that should be at the site root is not uploaded.
