CSS Form Builder is Stuck and not sending the email

Hi,
I've had this form working once before but something happened while I was trying to make updates to it and now it's not working correctly.
It validates OK but gets stuck when it's time for it to go to the thank you page and send an email out...

What am I missing?

license_form1Web2.php
Above is the web address for it and I also attached it.

Thanks in advance

but gets stuck when it's time for it to go to the thank you page and send an email out...

can you provide some more details what you mean by that? does it show an error message? if so what is the error?

when i open the page in Dreamweaver, it gives an error that there are spry elements being validated that no longer exist, you should allow for the lines validating elements that no longer exist to be removed.

also try deleting the webassist/email/mail_php.php file, then editing the UE wizard to have it regenerated

Nope that didn't work...

Hi Jason,
I tried what you suggested and it didn't work and I'm not getting any error messages either.

But when I do a link check on the form it seems to be having trouble finding the validating and mail php documents needed.(../webassist/forms/wa_servervalidation.js and /license_form1Web2.php)
Also I Just tested the form online and it doesn't seem to be validating at all-oops!

How do I fix this?

PS I'm working in Dreamweaver CS3 and CSS Form Builder 2.
I haven't been able to up grade yet.

Attached is the latest version of the form
