connect error from data assist form to mysql

Hello~

I've been building a registration/login series of pages with data bridge. All the pages are functioning (registration, login, logout, and forgot password). However, I'm getting this error from the userupdate page:

I am currently testing the page on my local server.

Warning: strpos(): Empty delimiter in /Applications/MAMP/htdocs/ProWaterParts_website_2013/pwp_pages/Connections/ProWaterParts_Store_local.php on line 15

Warning: Cannot modify header information - headers already sent by (output started at /Applications/MAMP/htdocs/ProWaterParts_website_2013/pwp_pages/Connections/ProWaterParts_Store_local.php:15) in /Applications/MAMP/htdocs/ProWaterParts_website_2013/pwp_pages/webassist/security_assist/helper_php.php on line 77

I can't find any reason for the first error - the php require_once code for the connection on this page is the same as all the other pages in the series. Any suggestions of what I should look for?

Hmm.... I still don't see the problem....

This is the code that either Dreamweaver or Webassist generated and it's working with the other registration pages. Why isn't it working for this page? And how should I edit the code?
