File upload not working

File upload not working

I have trying to get a multiple file upload to work, but getting no luck.
The file names are not entering into the database, and no files are uploading to the folders referred to.
I have attached the page in question.
The debugging information provides nothing useful (attached).
Folders are set to 776 on the servers.
php version is 5.2.17
Any ideas?

for file upload to work, the forms encoding type must be set to multipart/form-data

change the form tag:

<form  action="property_Insert.php" method="post" name="WADAInsertForm" id="WADAInsertForm">
