JS error - file missing it seems

Hi,

I've been receiving an error when trying to use various design extender extensions - see the attached screen shot.

I have deleted the cache file and reset the workspace several times, still no luck. I looked in the directory location the JS error referenced and couldn't see the formInsert.js file mentioned, its as though the file is completely missing.

I'm using Dreamweaver CS6 (creative cloud subscription) on 64-bit Windows 7 OS.

Anyone able to help with this?

Seams Adobe made an update in the 12.1 creative cloud update that effects how forms work. See post 8 and 9 of the following thread:
