DA Update page failing to update

Hi I have an item update page with a recordset to pull and present the item selected on the previous page, into my update page form.

The record calls up as expected and i have my form fields populate correctly.

If when the user changes an item in the form and saves, the updates fail to save to MySql DB.

I've stripped back everything to the essentials, but struggling to establish why.

Update form attached.

Any ideas please?

Thanks.

change the action of the form:

php:
 action="<?php echo (htmlentities($_SERVER["PHP_SELF"], ENT_QUOTES)); ?>"

so that is passes the Query string used to load the page when it is posted:

php:
 action="<?php echo(htmlentities($_SERVER["PHP_SELF"], ENT_QUOTES)."?".$_SERVER["QUERY_STRING"]); ?>"
