Problem with dynamic field

I have a table where one field is dynamically linked to another table. In DataAssist, I created the pages, Insert, Update, Details and Results. The Results, details and update page are fine. The Insert Page is blank. Any clues?

a blank page means that a php error is occurring but error reporting is turned off.

to turn error reporting on, add the following code at line 1:

php:
<?php

error_reporting
(E_ALL);
ini_set('display_errors','on');
?>



once we know what the error is, we will be able to troubleshoot the cause.


NOTE: When attaching files, please compress the PHP file to a zip archive and attach the zip file.
