CSS Form Builder isn't prevewing

Please, I'm having trouble previewing "Form Details" as this is how it displays in my "CSS Form Builder Extension," in DreamWeaver CS5 and in my browser from the file attached. The Basic info about my computer is as follows:

Hp - Window Vista Home Premium
Processor: Intel(R) Core(TM)2 Quad CPU Q9000 @ 2.00GHz 2.00GHz
Memory(RAM): 6.00GB
System Type: 64-bit Operating System

I'll be glad if anyone out there can help solve this problem, thanks!

What version of form builder are you using?

What other extensions (including version numbers) are installed?

The extensions are three in one and their versions are:

Design Extender 1.0.9
eCard 5.0.6
Data Bridge 1.0.5

The above extensions are all I've got installed in my Adobe Extension Manager CS5.

updates to Data Bridge and Design where released yesterday.

Uninstall Data Bridge and Design Extender from the extension manager, then install the newer versions that are available in your Download center.
