Incorporating DataAssist forms into existing page

The opening page in help says:
Note: Prior to launching DataAssist, you should have first defined a site in Dreamweaver, and opened a PHP page from that site to insert your form to.

But my DataAssist forms do not appear in my open php page. I didn't see an option in DataAssist to prevent it or cause it.

What's the procedure?

The suggested technique is to run Data Assist and include the form on the page with a Framework builder plugin.

If you don't have Design Extender or Framework Builder, then your only option would be to copy and paste the Data Assist code from the created page onto your existing page manually.
