the primary items for sale are the opportunity to serve as a crew member on a tall ship in the re-creation of the Battle of Lake Erie in 2013. Many of these sales are gifts for family members.
I need to be able to insure that if it is a gift the purchaser is induced to include the recipients name because a certificate bearing his name is sent to that person, and their names will be engraved on a permanent memorial plaque.
A yes/no checkbox with a required text field if yes.

Any thoughts on how I might proceed?
