JavaScript Error

Latest version of CSS Form Builder and also applied the patch.

I have an Upload Files (photo1) (prior version) behavior. When I double click I can open the File Manipulation: Upload Wizard. However, the File field is blank. So I click the drop down to choose photo1. When I click the Next button I get a JavaScript error. I have already tried deleting cache. See Attached.

what other extensions do you have installed including version numbers. It sounds like you have a conflicting extension installed.

To add... It seems like I have had to double purchase extensions. Why don't you all list what extensions have been combined? Your sales are very confusing and the documentation is extremely out dated.
