Insert Record not working

I've created a form which can be found at apply-1.php and applied Universal Email and File Manipulation to it so that it will insert a record into a database, upload the attachment and email the form along with the attachment when a user completes it an submit the form. It is an employment application.
I have uninstalled the extension twice and attempted to get the insert record to work from my desk top and laptop and am not able to make it work.

would you advice that i use the built in DreamWeaver insert record and then apply the UE server behavior to accomplish this?

neither universal email or file manipulation will insert the form informtion to a database,

you either need to use the Dreamweaver Insert record behavior or use the Data assist insert record behavior to insert the form information to the database.
