Form Validations problem

I can't see the data after the first form validation code, right after the field 'First Name'. The code was generated automatically by the form toolkit. I don't know what's wrong with it. Attached is the file. I appreciate any help. Thank you.

the server validation behavior is not applied to this page, it looks like after you added the initial value to use the validated entries collection, all of the code in the head got deleted


you will need to re apply the Server validation behavior, then re apply the validated entries as the initial value.
