Create Forgot password Page is missing email field

I am trying to create a forgot password page using the Create Security pages feature. The page that is generated does not include the email field. How do I get it back?

This solution to another problem also solved this problem:

I was mistaken. The fix I mentioned did not correct the missing email field problem.

I added an email field but the form still doesn't work. It is not producing an error when I put in an email address that is not in the database. it just returns to the forgotpassword page. Nor does it send an email when I put in an email address that is in the database.
