Error when displaying form page

I got a similar error on the last site I built with Security Assist and CSS Form Builder. Tried the solution you gave me that time but still getting this error:

Warning: require_once(webassist/email/mail_php.php) [function.require-once]: failed to open stream: No such file or directory in /homepages/46/d366494719/htdocs/myoscript2/contactUs.php on line 19

Fatal error: require_once() [function.require]: Failed opening required 'webassist/email/mail_php.php' (include_path='.:/usr/lib/php5') in /homepages/46/d366494719/htdocs/myoscript2/contactUs.php on line 19

I am attaching a copy of the PHP file.

this error means that some of the files from the webassist/ folder are not getting uploaded to the server.

make sure to upload the entire webassist/ folder to the server.
