PS3 - Contact US Email Issue

I'm getting the following error when I go to my contact us form and submit.
contactus.php

Parse error: syntax error, unexpected ',' in /home/content/25/8160825/html/0_store/WA_Universal_Email/Email_Templates/contactus.php on line 14

Unsure how to resolve, please help!

Hosted by Go daddy! tried leaving email server blank in Admin and entering smtpout.secureserver.net
Neither clear the error message above.

I found the solution to this issue from a previous submission.
