Weird Error when form submits

Weird Error when form submits

I get this error when my form submits and my thank you page does not come up..........The code is the same code I use over and over for many clients and now it is wigging out. Not sure what to do, I carefully compared working code to this form code and I see nothing that has changed.
Any ideas?

Here is the submitted content that I do receive even though the thank you page doesn't show up. Could it be that I am using a custom submit button(the only difference) and therefore that is why there is the submit x and y? How do I get rid of the xy?

I'd like to see a copy of the contactus.php page giving the error. Most likely it is just a blank line of code or space that needs to be deleted on line 7.

if you need further assistance, we would need a copy of the contact us php page so we can see the code, compress the file into a zip archive and attach that your reply.

Ray you were right, there was a space above line 7, removed it and all is well. Also removed the value and id names on my submit button which removed the extra xy submission fields.

Ahhhh, I can relax now. Thank you for pointing me in the right direction. I have made a note of this problem to remember it in the future.
