Cannot get security assist 2 to work - internal server error 500

Hi,

I'm having problems getting a log-in page to work with security assist 2. I've run through the wizard to create log-in and log-out pages and no errors occurred. However, whenever I attempt to view my page in a browser I get the dreaded 500 internal server error.

I've manually uploaded all include files and double checked they're all in the right place on the server, but I can't get the page to work. I have also tried turning PHP error reporting on but I still only get the 500 internal server error.

I've attached my log-in page. Can anyone help - is there something wrong with the code created by Security Assist 2?
