Form Builder 2.0 form build errors

Form builder will not create form.
I have removed all older extensions, UE, Html editor etc as per posted installation guide.
I have removed and reinstalled Form Builder a number of times, deleting the Maxfilecache so many times I have a shortcut to the file.
I have replaced the Shared.js file with one Created Thursday, 6 January 2011 10:37 AM.
This was posted on the forum.
It will not work no matter what I do.
Using DW CS5, Mac Pro OS 10.6.7
"Type Error" ShowIfObj.setparams is not a function on different "Build groups" See screenshots, just 3 of the many errors.
Even the simplest supplied contact template have multiple errors for each field.
How do I fix it?
Can you help?

I have replace the Shared.js file from the web fix known issues again as I did prior to my last email.
See my previous email.
I have attached a copy of the form page I tried to create with only a simple form but still errors.
No matter what I try I cant get it to work

Having the same issue

I updated with the fix and now its really doing weird stuff. It won't add certain things..like calendars or preset validations for the contact form. What did you find out? Is there anything I can do?

Form builder

It has been a while since I viewed the forum reply but I still do not have or cannot find a support ticket re this problem in my support history.
I still have not resolved the problem.

CSS Form Builder JavaScript file missing

I created a new PHP doc and clicked on the CSS Form builder icon.
An error message prompts.

The following JavaScript error(s). Occurred:
unable to open script file ".../.../WebAssist/RechTextEditor/iRiteShared.js"
(error 3)

That file does not exist. Do I need to download the file again? or will be it provided?
