Cannot apply progress bar to a form

I have the progress bar working on an existing form.

I later upgraded from Secuity Assist 1 to Security Assist 2 and now get an error message pop up in Dreamweaver when trying to apply the progress bar to a new form.
The error says "Error in initialiseUI ReferenceError WAFormBuilder is not defined"
If I click 'OK' the second window pops up - but nothing then happens.

I am using DWCS5 on Vista SP2,
Forms Toolkit, Universal Email 4, Security Asist 2, Data Assist 2, HTML Editor are installed.
I un-installed eCart5 and Cookies toolkit - but the progress bar icon is still showing in the DW (WebAssist) toolbar - so I assume that it should still work.
I have deleted the cache file.
