Index error in drop-down search field

Dear Support, I've created a Database Management system using Data Assist and the supplied BlueSkyMusic database. I've added search options on the results page. However when I am in the results page and I wish to do a search on a drop-down field say 'category' - the following appears in my drop-down "Notice: Undefined index ItemCatID in C:\wamp\www\sandbox\Results.php on line 275>CD." I have attached 'screenshots' of the results page and the relevant code for the field. Can you spot anything? Everything was generated by DataAssist.

make sure that the recordset used for populating the list returns the ItemCatID column

if you need further assistance, send a copy of the page in a zip archive so I can see the entire code in context

Jason, attached is zip file for you attention as requested. If you do a search for say Cd's it will bring you to the results page, which contains a search page at the top of the results. In the "ItemCatID" field the error appears.

edit the WADAitems recordset so that it returns the itemCatID column, right now, it does not return that column.
