CSS Form Builder 2 validation- client & server issues

Hello, I am having several issues with form validation after upgrading to Form Builder 2.0. I already owned all feature individually,(val toolkit, DFP, UE4, HTML edit - all removed for the upgrade) Now after upgrading I can not add ANY client validation to existing forms and I can not get multiple server validations to work (ie. file size and extension verify). I am including a screen shot of the client validation wizard. This show that form fields are not detected, therefore preventing me from being able to add any validation. please help. I have already tried all of the suggestions I could find on the forum including clearing the cache file and reinstalling the extension.

Here is one of several pages that do not work. You will see that there is 1 server vslidation but if I try to add another it breaks the page.

add a name to the form tag.

change:
<form action="" method="post" enctype="multipart/form-data" id="GROform">

to:
<form action="" method="post" enctype="multipart/form-data" id="GROform" name="GROform">

Jason,
I did as you suggested. That did allow me to see the form elements and add a validation. I tried to do a File Extension validation. It added it to the page but when I test it all files are being rejected and failing. I tried removing the (.) from the extension as noted elsewhere but it doesn't matter. I also can not use the server validation wizard. I get an error message to configure a site and testing server in order to use dynamic data. This is already done so I don't know this happens. see the attached capture of message.
thanks,
Stephen
