Trouble inserting multiple records using data assist

I need to create an insert form that will retain primary user data for multiple record entries.
IE... a user would enter name, employee number one time,
then, using the repeat region... specific details (up to 30 records) such as date, time, details are entered into the database as separate records on a single 'insert'.

Goal: The database would contain one record per date without requiring the user to retype repetitious user data.

I have the insert form set up and data is flowing into the database. The issue is that the repeat region/insert multiple records is inserting into a single record using the 1,2,3 format (that is, data entered into the repeated fields is being grouped as a single entry, rather than creating three separate records).

HELP! I'm still using Dreamweaver 8 and am wondering if that may be a part of the issue. I've attached a file for your reference.

can you compress the asp file into a zip file rather than past the code in a pdf please.

DataAssist 2 is not compatible with Dreamweaver 8, it is compatible with Dreamweaver CS3 and above.

Yes. Many of your tutorials say that Dreamweaver 8 is the minimum requirement. I ordered it based on a printed tutorial. Then, I notice on the requirement of CS3 or higher at the download. I was hopeful that I could nurse it along, if not I'll have to return.

the repeat selection code on this page is not correct, there should have been a hidden form element added and the repeat text box name should have been changed to a dynamic name. I cant say if this is because of the Dreamweaver version or if the code was altered after the repeat selection was added.

Thank you for taking time to look. I have spent the better part of two days upgrading and reinstalling. I upgraded to Dreamweaver CS5 and the multiple record insert works perfectly.
