Results page is blank

I've created a UE object file on an existing form page (attached) using Linux per web host instructions. When the site is online and form is 'submitted', the results page is blank. The page views fine after deleting all the WAUE code.

Also getting a dreamweaver error when trying to upload webassist folder and web files to the server; but when loading only webassist folder, no error. The error message cites server permissions regarding the webassist files.

Attached are home page with link to form page, form page, results page and checkout page with object files.

Thanks

Is it the formresults.php page that displays blank?

I see a problme with the following code on that page:

php:
<?php session_start();

$_SESSION[customeremail]="$_POST[customeremail]";
$_SESSION[customeraddress]="$_POST[customeraddress]";
$_SESSION[NWDallas]="$_POST[NWDallas]";
$_SESSION[NDallas]="$_POST[NDallas]";
$_SESSION[SDallas]="$_POST[SDallas]";
$_SESSION[Income]="$_POST[Income]";
$_SESSION[Age]="$_POST[Age]";
$_SESSION[Listmenustate]="$_POST[Listmenustate]";
?>

it should be:

php:
<?php session_start();

$_SESSION['customeremail']=$_POST['customeremail'];
$_SESSION['customeraddress']=$_POST['customeraddress'];
$_SESSION['NWDallas']=$_POST['NWDallas'];
$_SESSION['NDallas']=$_POST['NDallas']
$_SESSION['SDallas']=$_POST['SDallas'];
$_SESSION['Income']=$_POST['Income'];
$_SESSION['Age']=$_POST['Age'];
$_SESSION['Listmenustate']=$_POST['Listmenustate'];
?>

iof your still having problems, try adding the following at line 1 of the page that is showing blank to turn error reporting:

php:
<?php

error_reporting
(E_ALL);
ini_set('display_errors','on');
?>

if Dreamweaver is showing errors about permissions on the server, try deleting the webassist directory from the server, then uploading it again.

javascript error message

Got the blank page error worked out. Original UE files have been deleted, and am starting over. Now I get a javascript error (exclamation '!' next to server behavior for object:

While executing applyServerBehavior in WA Universal Email.htm, a javascript error occurred.

Updated files are attached.

Thanks

delete the Dreamweaver cache file from the users configuration folder:

Blank page again when UE code is added

Deleted the config.

Still getting blank page where object code is added. Page ok when code is deleted.

Do the UE object files go in the same httpdocs as the web pages? When I pulled the folder out of there on my local folder, the '!' disappeared. However, no matter where I put the WA folder on the remote server, nothing works.

Still getting this error when uploading the form page with UE script in it:

httpdocs:form.php - Put operation successful
/WA_Universal_Email/ - error occurred - Unable to create remote folder /WA_Universal_Email/. Access denied. The file may not exist, or there could be a permission problem. Make sure you have proper authorization on the server and the server is properly configured.

Files are attached.
