Validate radion buttons

Is there a tutorial to apply validation to radio buttons, I've searched the forum but nit found a basic instruction.
When I try to apply client side validation I have three choices for two buttons, if apply validation to any the form fails.

I read that I need to have required on one field and conditional on the other two, but I have no idea how to do this.
I have attached the file.

Just make sure that the radio buttons have a value:
<tr>
<th>Membership</th>
<td colspan="3"><input type="radio" name="memb_typ_usr" id="memb_typ_usr" value="12" />
<strong>Level IM-1: $12</strong>
<br>
<input type="radio" name="memb_typ_usr" id="memb_typ_usr" value="16" />
<strong>Level IM-2: $16</strong></td>
</tr>


and use required validation. Select the first radio button in the list. There is no need for conditional validation.
