Problem with attachment

Hello friends, I have a problem, I tried to make a file upload form, but something is wrong. I used the techniques with CSS Form Builder and Universal Mail 4. And when I try the form does not work. You can see this on the following link:

denunciarte_enviar.php

Similarly here I send in a zip attachment with the code of the page, to see if they can help me.

this file has not been configured for attachments.


In the universal email wizard, click the paper clip icon to configure attachments.

on the attachments tab, select the "Submitted File" option, then select your file field from the list.

Problem

I precisely followed all the steps and the corresponding tab did that to me says, but just does not work! every time I re-open universal mail reappears as not selected, that another way I can configure it manually?

do you have the waue_denunciarte_enviar_1.php file open in Dreamweaver when you edit the universal email settings?

Make sure that this file is not open or the changes will not take effect.

Also make sure this file is not set to be read only.
