CSS Form Builder won't install on CS5

Hello, I have downloaded the new version of form builder and when I try to install into CS5, I get this error:
The extension package is invalid. The extension will not be installed.

I re-downloaded and tried to install from the fresh file, but the same error occurs.

I deleted validation toolkit before installing per instructions, but that did not help.

Any ideas on how to make this work?

Mac snow leopard, Creative Suite 5, Mac Pro Desktop, 10 gigs ram
