Using Radio Buttons with multiple insert

Can someone explain the right way to use radio buttons in a repeat region for the multiple insert in dataassist? This is not working for me at all.
I am trying to update the answers to a quiz, yes updates to 5, no to 0, and it is 25 questions that load from a recordset. Seems that the form believes all radio groups in repeated region is just one.
I tried with checkboxes, althought these are not mutually exclusive and the wrong choice for me, but the update does not work regardless.

any help?

How have you applied the repeat selection server behavior to the form fields? When applied the repeat selection should give each radio group a unique name. Please post back with the page in question so that we can take a look.
