Unable to Update Image using File Manipulation

I have spent nearly the whole day trying to resolve this issue and have followed the pdf guide with no luck at all.

My insert page works with no problem as does the update page (text) however the file upload does not happen and the file name is erased from the database.

I have attached the file below

1) for file upload to function the forms encoding type must be set to multipart/form-data

From the tag selector, in Dreamweaver, select the Form tag. In the property Inspector, change the encoding type to multipart/form-data.


2) You have named the file field and the hidden form element with the same name, your form elements must have unique names, You will need to rename the hidden form element.

3) Bind the Default File option to the hidden form element after renaming it.

The field names was something that I had changed as a last ditch effort, but the bit that I had been missing was the form type.
