The date picker is not showing up.

Please advise.

One thing to look into is the fact that you are calling two separate version of jQuery on your page. You have an earlier version being called from the Google Server which appears to be for the menu and then a later version that is called by the CSS Form Builder generated code. You may want to just call the latest version of JQuery PLUS the Jquery UI both from google and then delete out the form builder's pulls. If you do decide to pull both from google, be sure to use the latest versions and replace the one you have for the menu.
