Just making sure... did you upload the folder marked "webassist"? While it seems simple, many times, that is the problem.

If that's not the case, could you attach the page with code so we can actually see what's going on?

As Brian suggests, please add the php page to a zip archive and send us the zip file so we can examine the code to see what may be going wrong.
