File Upload on Registration Form

I have added a file upload on registration form. I loads the file but it does not write the name to the database.

I tried to open the Insert Record on Server Behavior to give the Server Behavior name to the image file but it won´t show up as it does on other pages that I have done before.
I´ve created the pages with Security Assist, somehow it does not even let me ecript my passord as I would like to.

The security assist pages created buy the wizard use the built in Dreamweaver insert record server behavior, you will need to remove the Dreamweaver insert record server behavior and swap it out for the Data Assist insert record server behavior.
