data missing from a form created by CFB

Hi all,

I have set up a form using CFB which has menu/list. I am receiving other infomation such as name and phone number but information used in a menu/list is missing in the email I receive.

And also a shot of what I get in my email with the parts that have a yes / no (menu) not populating.

The email I receive from the contactt form is missing all the yes / no answer.
