HTML to PHP contact form

This is what Id like to do...

I have articles posted on my site daily and I would like to add a contact form at the bottom of each article so that users/readers could comment. I do not want their comments to posted on the page or anything just sent to an email just like the contactus.php would... However I do not want these article pages to be in php.

My goal is to have a form identical to the contactus.php provided, captcha included, and have the form results sent to contactus.php then sent to email if thats possible...

If I need to be more clear just say so, I am new at web design....

the captcha and security questions rely on server side code to work. They cannot be placed on a static html page, only on a php page.

If you place the contact form on a static HTML page, the server will not be able to process the PHP code in order to create the captcha image or the security question.

the contact form can only be on a php page.
