CSS Form Builder Always Fails Validation

We created a form using Form Builder. contact.php The form, even if it passes validation, when submitted it goes to the failed redirect page. I have rebuilt the page numerous times with the same result. Please see attached files

It definitely seems like one of the server validations is failing. To determine which one add in the following code to your page. Make a new line just after line 28 and add in this:

php:
die("the validaitons that are failing are: " . $WAFV_Errors);



This should print out a message letting you know what validations failed on the page by their numbers. Please post back with this info so we can determine what the issue is.
