Photo Contest Submission Form

Hey Webassist Form Guru's,

I have a photography contest that needs submission form. The form will give different options depending on who is entering the contest (professional, amateur, student, etc.). Each submission will allow 7-12 images to be attached and will require an essay (250 character limit). This form must capture all data using php/mysql. And send email notifications.

Most of the form is done and a clean database setup (php/mysql).

Please, experienced people only.
