Log In to Logged in Page Issues

I have created a log in page using security assist. When I attempt to log in, that log in page displays the following:

I have checked all links, database connection, etc. cannot seem to resolve the issue. Thoughts and guidance would be appreciated.

These error messages are all referring to your connection information. If you have your connection file included at the top of the page and still have these errors it is probably due to a mismatch with the variable names in this file and the names on the page. Looking at the last error it could also be that you have the wrong username and/or password specified.
