Need Help w/Contact Form

I'm trying to create a form for my client. However, they are hosting the site so I do not have access to any of their hosting company's server side form tools. This is why I purchased your "Contact Form" (plus I liked the validation aspect). That said, I cannot get it to work. I do not even get the Thank you page. The link below is on the testing server. I just used the form out of the box but added the template. For the final form I would like to add more fields and check boxes. I'm sure most of the problem lies with my inexperience with your product, so any help you can supply is greatly appreciated.

Have you tried uploading the page by itself without the template being applied? It seems that the page is posting but not triggering any of the server side functionality. If you can test it by itself this should help to let you know if it is the code on the page or the inclusion of the template that is the problem.

Contact Form Not working

Eric, thank you for your response. I uploaded the ContactForm_GMC folder without applying the template. I places me email address in the code of the WA_Globals file. The form does submit, however, I receive the thank you page but not the email and it will also submit without entering any information, so for some reason the validation is not working. The site that I'm creating the for m for , was created with SiteAssist.
