Data not in emails

I have set up form builder and have inserted universalemail. When I test the page, I recieve an email but no data from the form. This was a simple contact form and can be seen here contact.php

can someone point me in the right direction...?

Can i ask how did you add the form elements to your Email?

If you could tell me the steps you took, we might be able to find the problem.

Ok...I think I see the problem. But have another question. I need to put code for php in the last tab in universal email. Under the body tab I have to insert code. But where is it and what do I insert? I have name*, email*, phone, comments* and a call back check box.

On the last tab, you should a DW button, clicking the button will open the page in DW and allows you to add your from elements by using the Bindings Panel (Window -> Bindings). Once the Bindings panel is open you can drag each form element to the content area.
