Basic Help

I'm new at this and need some very basic help. First when I install a form using this extension everything goes fine until I try to view it and then I'm asked to set up a server. I'm using DW CS4.

In order to create a contact form that ultimately will be sent to an email address what do I need to do? I'm hosting sites with HostGator, but I want to be able to view the form on my local machine, do I need to install a testing server like WAMP? Do I need to define a SQL server as well?

What is the simplest method to create a form and have it emailed?
