Can't Add Pictures to Long Description in PS 4.5

Follow Up to Problem with Adding Graphics to Long Description

As an addition to the previous question, I noticed that I was able to past some mixed text and graphics from word into the long description window. The text appeared to be copied properly, but all graphics elements were replaced with their "placeholders". You could right click on these elements, and select "Image Properties", but there didn't appear to be any way to then select a graphics file from a windows directory. If I clicked on the "Browse Server" button, a window appeared with the following message:

alert("MDB2 Error: no such table\nSELECT extension, plugin FROM kfm_plugin_extensions WHERE user_id=1")

I also noticed that if I went into a long description window for a product that I had entered with the PS 4.0 version, I was able to copy a graphics image into the clipboard and paste it into the long description that I was currently trying to build.
