Username Check before Insert

HI,

I have followed the suggested method of checking if a username exists prior to inserting a new user.

I believe I have followed instructions properly, however, whenever I add the username server validation to the php page, the insert of a new record fails completely.

The validation code seems to stop the insert proceedure altogether, whether or not there is a duplicate username present.

I have attached a zip file of the page.
Please let me know what I am doing wrong, I have been trying to get this perfected for 2 days, probably missing something obvious.
