Error Initializing Add Form UI - HELP!

I'm new to the WA SS and am trying to use the Form Builder. I've been having problems all day but have been able to resolve most of them via other threads and other common problems. HOWEVER a new one just popped up and I can't find anything on it.

I opened my php form (I had previously made with FB) and tried to "Edit Form" at which point I got the "Error Initiliazing Add Form UI: Reference Error: theServerLanguage is not defined" message. It wants me to add a whole new form which I do not want to do. It also won't let me actually use anything IN form builder now, like it's locked up??

I've tried restarting, I've deleted my .dat file... nothing seems to be working. HELP!

Also - on an un-related note, how can I pop this form built on a php page into my xhtml page? I'm new to PHP and have been very confused by everything I've found online.

AND - when I design it in the preview it shows up as a 2 column form, when I view it in DW CS4 it's a 2 column form, however when I open it in my browser it's a standard one column form... HELP!
