DFP/File Manipulation Does Not Upload Image

Hello WA,

Just purchased Universal email w/ DFP/File Manipulation. I have setup a form page here:
34stt6s

The page is to simply upload a single image file.

The form data loads just fine, but is no action is taken by DFP/FM "helperphp.php" script. The file does not upload to the designated (or any) folder, nor does the file name appear in the "upload_Name" data base field.

I've spent hours searching & trying forum suggestions with no success.

Any help is appreciated.
