few things to try for debugging this:
1) move the error reporting code:

error_reporting(E_ALL);
ini_set('display_errors','on');

to line 1 and have it on both pages.
2) change the session code so it only triggers when the form posts:

?>
3) on the form results page, add var_dump after the body tag:
<?php var_dump($_SESSION); ?>
